April member of the month April 28, 2017 SPRINGFIELD, Vt. – Josh Dufresne discussed SMCS’ model of healthcare, where patients establish a “medical home,” working in close partnership with their primary care clinician, integrating medical, behavioral health, and dental care, through a wholistic approach to care. SMCS integrates behavioral health in each of its primary care health centers, and dental services are available at Ludlow Family Dental, Chester Family Dental, or through coordination with patients’ existing dental providers.
